There are no safe versions of VaultWiki 3. See: https://www.vaultwiki.org/articles/158/

Please note that since VaultWiki 3.x received its End-of-Life almost a year ago, there is no patch for that family or earlier, and it is now no longer available for download.

I suppose it's a bug that the download link still shows. There should not be a download link for VW3 anymore.

For vb3 I still prefer VW3 over VW4. VW3 has had a long period of bug fixes. It is the only version of VW that has had a good number of bug fix releases.
Naturally VW4 is in constant flux with new features being added with every release. This means that new releases introduce new bugs.
Once VW4 reaches a level where it has no regressions, enough new features to outweigh the disadvantage of loosing VW3 'new posts' function and has had a good number of pure bug fix releases, then it will be the time to move a vb3 site to VW4.

In regard to the FILE bb-code: just disable it if you have it enabled. I have never used it on my site. VW3 has a few known bugs that unfortunately do not get fixed, but overall its very stable. Its used on several big boards for many years. Any bug in VW3 has been reported long ago.
