It depends on the "collation" of the source database whether the username matching is case sensitive. This is not something VaultWiki has control over, or at least is not something it should, since the source database should be considered read-only for import purposes.
For fake guest usernames, it will use the same username as the source database, it just won't be linked to a valid user account on the forum.
VaultWiki will import file versions if you have file versioning enabled in VaultWiki. Options > VaultWiki: Content Types > Track Attachment History? If you don't have this turned on, then VaultWiki has no use for that source data and it will be ignored.

VaultWiki has a GALLERY BB-Code tag that works similar to MediaWiki's Gallery tag. However, VaultWiki will also use a scroller if the gallery meets some other conditions. Example: https://www.vaultwiki.org/screenshots/

Yes, MW namespaces are imported as both VW prefixes and VW areas to keep the imported articles named and organized in a way that is similar to the original wiki.
However, you can move your imported articles into other VW areas later or change the prefixes and they will still function the same way.
This bug was reported on the same day as VW4 Gold was released. There was not enough time to add a fix to the same release.
If memory serves me, the only phrases necessary for translation before the import are the vw_mw_ phrases, which should be changed to the appropriate MW namespace names.

is it possible to use vaultwiki for interwiki-links?
To perform an Import we need only a fourm admin with wiki admin privileges, as long as you set the import config file properly beforehand. If there are problems with the config, or part of the import needs to be rolled back, then FTP access is needed to make corrections.
VaultWiki does not support inter-linking at the current time, although this would be somewhat simple to implement in the future by extending the prefix functionality.
Target + Source are not on the same server - is this a problem?

Yes, this will be a problem for images and files. The directory containing this data should be downloaded and copied to a local directory on the target server before attempting the import.
