[4.1 RC 3] Breadcrumb inconsistency with a small number of page actions

Most page actions are rendered in the follow style:

Home -> Wiki -> Area -> Page name
Page name Page action

For these actions, there is also a matching page title with the action appended:
Page name - Page action | Site name.

Example breadcrumb:

Home -> Wiki -> Demo -> Testing playground for XF2
Testing playground for XF2 Version history

Example title: Testing playground for XF2 - Version history | VaultWiki - Wiki for XenForo Forums.

A small number of page actions do not follow this breadcrumb and title style.

On one page checked at https://www.vaultwiki.org/xf-wiki/, the inclusion of the tab name was observed as follows:

?do=source: YES
?do=history: YES
?do=promote: YES
?do=book: NO
?do=report: YES
?do=protect: YES
?do=translate: YES
?do=disambiguate: NO
?do=delete: YES
?do=redirect: YES
?do=watch: YES
?do=edit: NO*
*A unique case where the breadcrumb level is correct but the page action is not appended. This is probably because the action is shown in a dedicated h3.block-header element. I am indecisive about whether the block-header should be moved to vw-title-after for consistently or leave it as-is. Either way, the page title (as defined in <title></title>) should have the page action appended, e.g.: Page name - Editing page | Site name.
