      This is not a bug. Googlebot attempted to crawl your site while you were uploading a new vault/core/model/vw.php (probably while upgrading). There are three ways to prevent this:
      • Tell Googlebot to stop crawling while you are upgrading
      • Use symlinks on your normal installation directories, and use a staging area for upgrades. Then update symlinks once files have been all uploaded to the staging area. Doable but may be unrealistic in certain situations, such as patching 1 file.
      • Disable the VaultWiki add-on while uploading files. Re-enable after upload is complete. This is the method the upgrade manual page recommends.

      Otherwise it is possible for users and bots to access a file while it is being uploaded; that's just the way it is. There is no way to prevent that from a software perspective.

      If I use the mobile friendly test tool then I can replicate this error at any time:

      I am using a xenforo media gallery item page to test.

      This is because your robots.txt is blocking Googlebot from accessing VaultWiki's static files which are located in /vault/resources. You have a rule which denies access to /vault. In 4.0.x, the rule should only block access to /vault subdirectories which are not /vault/resources.
