Cannot reproduce. Make sure you aren't using an add-on called "BB-Codes and Buttons Manager". All versions of that add-on break VaultWiki's BB-Codes. The author is aware of the issue and I believe the next release of BBM will have the fix.

EDIT: I see you just made your post more specific. I will see if I can reproduce now.

I am not using that addon. It only happens on wiki index.

Still cannot reproduce on the wiki index. When you go to the editor for the index, are you seeing the full toolbar? If there's no toolbar, then BB-Codes are not allowed on the index.

Also, if you changed the BB-Code settings for the index after the last time it was edited, changing the settings does not rebuild the post cache. Try rebuilding the cache in Integrity > Counters.

After editing the page, the content appears normal on your site. I suspect that somehow you had a cached version of the page from when BB-Code was disabled. Also, you currently have page_img disabled, so images are not working. If you change that, you would have to edit the page again or rebuild the whole cache.

For easier debugging without disabling the cache, you can always see the uncached version of a page by adding the nocache=1 URL parameter. I will leave this open for now, since I think it would be more intuitive if changing the parsing options for the index or an area invalidated the relevant caches.

Marked as fixed in the next release. As of RC 3, changes to parsing options will automatically invalidate the post cache.
