Islands in Categories Have Blank Heading

When anonymous/island pages are added to categories, they all appear under a blank heading. They should be sorted by the first letter of their titles too.

More generally, a separate field than itemkey should be used for sorting, since itemkey is used for finding. Anonymous pages, although not findable, should still be sortable alphabetically. This change will take a bit more time as it requires changes to any queries that sort by itemkey so that they use sortkey instead.
