Moderated Items that Don't Exist

Whenever an attachment gets created, the counter for moderated items goes up and notifies all relevant moderators, even if the attachment is not moderated. Visiting the ModCP reveals that no new content is awaiting moderation, or that there is fewer new content than the counter implies.

Fixed in the next release. The counter will be fixed too.
