History does not display all avatars

do=history displays avatar boxes in different sizes and does not display all avatars. In many cases only the xenforo placeholder is displayed.

It seems that some edits (and possibly some other content) were not properly attributed to some users with CamelCase in their usernames, which means that some edits are treated as guests (guests have no avatars). I have found the cause of the problem and will work on a script to correct the attribution on your site (upgrade script for others). The issue affected non-MediaWiki imports.

This issue should be fixed by the upgrade script to 4.0.19. Closing this issue.
