Issue: How to use TOC outside of VW?

Historically, TOC was never made to work outside of wiki content due to design issues in the post cache of regular posts (except in VaultWiki 3, where an option was chosen to use the wiki parser for certain regular posts). There are other ways to handle it, but none that are currently in core. There are a number of BB-Codes that do not work in non-wiki content for similar reasons:
- TOC
- FOOTNOTE
- REFLIST
- TEMPLATE (and template-related BB-Codes)

For TOC, FOOTNOTE, and REFLIST, it is relatively easy to add support, but required some outside-the-box thinking before we realized it for things like WIDGET and GALLERY.

For TEMPLATE, it requires a lot more work.

I have many quite a lot of forum posts that utilize TOC in vb3/vw3. I need to keep these working after upgrade to VW4+XF.

Fixed TOC in non-wiki content in the next release. Tested and working in vB3, vB4, and XF.
Fixed FOOTNOTE/REFLIST in non-wiki content. Fixed footnote highlights in XF.
Fixed the deprecated forms FORUMINDEX, SHOWTHREADS, and BOOKINDEX in non-wiki content. Previously only the new WIDGET form was working outside the wiki.

I think that covers it. As I said above, TEMPLATE is a separate case and requires a lot of work.
