      Error received during automated and manual upgrading

      Code:
      During step 6 of upgrade I get this error:  Fatal error: Call to undefined method vw_DBalter_MySQL_Controller::fetch_field() in /home/stormtrack/public_html/vault/core/controller/dbalter/mysql/vw.php on line 423

      vw_DBalter_MySQL_Controller::fetch_field is inherited from vw_DBalter_Instance::fetch_field which is defined in the file vault/core/controller/dbalter/vw.php
      If you are getting this error message, then you have not uploaded the 4.0.1 version of that file.
