All New Edits Count as Rollbacks

If a page was ever rolled back to a previous version, all subsequent edits to that page will show as a rollback to the same version in the page's history. This should count as data loss because it makes it impossible to tell when a page was truly rolled back if the page is later rolled back to the same version in reality.

Fixed the underlying issues in the DMs, but still need to write an upgrade script so that only known rollbacks are marked in the history. The logs of some true rollbacks will be lost, but I think it is better that a few definites are preserved rather than have a flood of fake rollbacks.

Upgrade script written. It's a little expensive (it basically scans all edits for each page/attachment that has a rollback marked as the current revision), but should be the most efficient way to get the job done.
