Old Attachment Versions are Overwritten
When attachments are stored in the file system, old edits have the same file path as new edits, which means that the new edits replace the old ones. This is not desirable behavior, since we can't revert the attachment. When an attachment is no longer new, it should be moved to a sub-folder named with the appropriate attachfileid.
Also, whenever the attachment is renamed, the location in the file system should be updated with the new path based on the title - even old edits should be moved.
