PHP 5.4 Support (VW4)

A lot of sections of VaultWiki 4 need to be updated to comply with Strict Standards in PHP 5.4. It also currently hits fatal errors due to some PHP features now being removed.

Marking this as complete. VaultWiki 4 has been running on PHP 5.4 for over a month now. Any remaining issues will be dealt with on a case-by-case basis.
