Multiple Discussions per Page

Allow multiple threads of discussion for one wiki page.

For example, we can have normal comments, Editor comments, and Off-Topic comments in separate threads to keep things neater, meanwhile they all belong to the same single wiki page.

Add an AdminCP page to create default discussions and apply these on a per-area basis.

Implemented for 4.0.0 Alpha 1.
