If you added extra languages after installing VaultWiki, while VaultWiki was disabled, or if there was some error, you will have to manually add the column to the database:
      Code:
      ALTER TABLE vb_vault_language
ADD COLUMN translate_3 INT UNSIGNED NOT NULL,
      ADD INDEX translate_3 (translate_3)
The likelihood of this happening is low, and this is already fixed in version 4 so I'm marking this as Won't Fix.

The upgrade to version 4 doesn't reference any columns in this table directly, so you can have the added column or not. Languages are stored differently in version 4 and this issue is avoided.
