      Inserting Images Doesn't work

      After upgrading and enabling vaultwiki, inserting images in CMS articles will not work. When you insert an image it doesn't let you see the image just shows attachment bbcode with text inside. After disabling vaultwiki to make it work my insert image button is now gone...

      This is a template error. It was working on our dev site, looks like we didn't merge it into the ZIP yesterday.

      In vault_edit_imagejs, get rid of:

      <vb:if condition="!VAULT_414_COMPAT">
      and

      </vb:if>

      Are you getting any Javascript errors in your browser? If you're using Safari or IE8, you may be experiencing Javascript errors, re: https://www.vaultwiki.org/issues/2360/

      That was fixed by simply downloading the full VaultWiki package again and replacing all files to make sure you actually uploaded them all but I'm not sure if it's so simple for you. Since that wasn't in the template, you might not have gotten the template changes in 3.0.12. Does the wiki editor load for you? Either way, I would also run /vault/install/finalupgrade.php again after replacing the files.
