      I experienced this a few versions back myself but it looks like it has been fixed since then. In Beta 5, there were changes to the rollback form template - so it may have been fixed then (even though the template was not marked as requiring a revert).

      Try reverting the following template: vault_inlinemod_rollback

      I have tried to revert the template but i can't the content is as follows:

      Code:
      <form action="{vb:raw threadinfo.url}{vb:raw session.vw_sessionurl_d}do=history&amp;action=dorollback" method="post" name="vbform" class="block">
      <input type="hidden" name="s" value="{vb:raw session.sessionhash}" />
      <input type="hidden" name="securitytoken" value="{vb:raw bbuserinfo.securitytoken}" />
      <input type="hidden" name="do" value="history" />
      <input type="hidden" name="action" value="dorollback" />
      <input type="hidden" name="oldid" value="{vb:raw revision.revisionid}" />
      <input type="hidden" name="rollstarttime" value="{vb:raw rollstarttime}" />
      <input type="hidden" name="url" value="{vb:raw threadinfo.url}" />

      <div class="postbit vw-content">
      	<h2 class="blockhead">{vb:rawphrase vault_rollback_confirm}</h2>
      	<div class="blockbody formcontrols">
      		<div class="blockrow">
      			{vb:rawphrase vault_confirm_roll, {vb:raw threadinfo.fulltitle}, {vb:raw revision.revisionid}, {vb:raw revision.revisioncount}}
      		</div>
      	</div>
      </div>

      <div class="blockfoot actionbuttons">
      	<div class="group">
      		<input type="submit" class="button" name="type[continue]" value="{vb:rawphrase continue}" accesskey="s" tabindex="1" />
      		<input type="submit" class="button" name="type[cancel]" value="{vb:rawphrase cancel}" accesskey="r" tabindex="1" />
      	</div>
      </div>

      </form>

      EDIT: This really only makes sense if you have modified VaultWiki yourself (hook vault_url_generate)

      Have you created plugins anywhere on VaultWiki, or have you enabled any of the following "legacy hooks": postbit_display_start, postbit_display_complete

      Something is erasing the URL or resetting $threadinfo - and it would probably be in one of the contentaction hooks.

      newthread_post_start, editpost_update_start are enabled the rest of the legacy setting are:
      Enable support for Legacy Wiki Markup?: Disabled
      Use the Legacy Special page Listings?: Yes

      Turned newthread_post_start, editpost_update_start off but still the same.
      Use the Legacy Special page Listings?: No but still the same.

      You still have access to my website so you can look for your self if you want to.

      Sorry it took me so long to track this down. This was a bug that someone fixed in the Beta 6 ZIP after its release without creating a proper bug report (which is why I didn't know about it). After reviewing your installation, it appears that you can solve this problem by downloading a new ZIP and patching your vault/tabs/special_tab_history.php.
