History Tab Revision List is Invalid XHTML

The History tab fails XHTML validation.

There are two instances of the URLs not being properly encoded in the History tab. One is in the history nav links and the other is multiplied because it exists in each bit. While we are discussing malformed URLs on this tab, the Contributor's profile link does not include the session URL.

Easily fixed. Updated tabs/vault_tab_history.php and template vault_history_versionbit.
