Book Subscriptions

When subscribing to a Book via the Watch tab, rather than just receiving emails about updates to the page itself, it should also inform you of updates to its chapters.

Working on this today. It required some reorganization and adding to the database to improve performance when sending the notifications by adding a lastupdate field, which is updated on new chapter, chapter edit, or chapter comment.
