Duplicate without user-selected canonical: /wiki/?title=X

Google is finding duplicate urls because somehow its seeing links to vaultwiki pages with this format: /wiki/?title=X
We only use this format: /wiki/title

Google is marking this as 'Duplicate without user-selected canonical' and therefore removing both the correct as the incorrect url from its index.

Confirmed that the rel=canonical tag is mysteriously missing.
