If this does not occur on all attachment pages, then the attachment being viewed is damaged, and this is not a bug. If the page has no content, you should attempt to delete it. When viewing the page, go to Options > Delete This Page.

I can download and open the attachment just fine. However, the MIME Type appears to be unknown.

Can you submit a ticket for this, and include a link to the attachment that triggers both issues? The description of the problem does not sound possible to me for only one attachment; perhaps each error is from a different attachment, and perhaps most attachments are affected.

Usually this happens if the vw_attachment record for an attachment is missing from the database, since the 'counter' index is a column on that table, but you would not be able to download the file in this case. For the 'filetypeid' index, this is a column on the vw_attachmentfile table, and would happen if the record is missing, but you would not be able to download the file in this case either. If the vw_attachmentfile record is present, it could happen if you are missing the 'filetypeid' column from the table, but this would make the MIME type of all attachments be unknown, and it would not explain the 'counter' issue.
