ToC placed in seemingly random location

Please see bug report 300959 on my live site.
The ToC has somehow been placed in middle of the intro, after the first sentence. Looking at the raw text / code I see no reason for this.
There are however a lot of weird bbcodes added by the importer. But its not clear why this would affect the ToC.

Fixed in the next release. This type of positioning (TOC after the first line break) is only expected if a not-malformed section heading cannot be located (usually the case with virtual section headings created by some obscure functions/add-ons or in special pages). However, the method was incorrectly used for all wiki pages.

In the next release, the expected positioning of immediately-before the first section heading should be used more widely.
