Inserting image file only inserts a link

After adding an image file to a wiki, it only inserts a link to the attachment page.

A member also tried to add an attachment and they failed as nothing happened. Please see thread:

Fixed in the next release. This occurs for newly created attachments. Basically any change to the attachment, such as approving if it's moderated, will resolve the issue.

To prevent it from occurring in more attachments, in vault/core/controller/dm/attach/vw.php, find:
