URLs in vw_revision_approved and vw_revision_deleted are not clickable

The url is not clickable with XenForo, may also not be with vB.
Also i inserted the page title as url link title

It is only safe to put BB-Code in the *_approved and *_deleted phrases. Other ones might be sent as emails where the BB-Code won't work. I should also note that the string $item[fulltitle][/url] might be confusing to the parser. It looks like something that would cause a fatal error in PHP 7. I think we would have to use another way than changing the phrase.

Fixed this in the next release by automatically converting URLs in the content of PMs sent by VaultWiki.
