[4.1 Alpha 2] Approval queue: Bad item title / posted by links, vw_deny phrase

1) Bad links:

Item title: URL has an empty href=""
Posted by: Username URL href links to wiki index

2) Phrase "vw_deny" missing phrase, did you mean vw_mod_deny?

3) The disabled textarea is pretty hard to read in FlatAwesome+ due to poor text/background contrast. Using readonly="true" instead might be better?

Fixed the above in platforms that were affected.

For XF 2.1, the approval queue needs a redesign anyway: https://xenforo.com/community/thread.../#post-1290683

In XF 2.1, made some changes to be more consistent with the newer the approval queue style.
